Send a Traditional Impression

  1. Verify the impressions for accuracy and quality. Please follow safety guidelines and pack your sanitized impression and bite in a biohazard or a sanitary Ziploc bag. If you are sending models, PLEASE WRAP IN BUBBLE WRAP before placing them in the bag, or they will break in transit! Any small parts should be put in a vial or an envelope. 

  2. Complete and sign the prescription form. Don’t forget material choice and shade! Please attach to the OUTSIDE of the biohazard bag.

  3. If you have multiple cases in a box, please staple the Rx to the matching impression. Please DO NOT put the Rx inside the bag! You can mail as many cases as you can fit in one box/bag using just ONE label.

  4. Refer to our RETURN DATE CALENDAR for the accurate date the case will return to your office. The Return Date Calendar has two dates in each box (the date you are sending the case (today’s calendar date), and the second date in bold is the date on or before the case will be back in your office! *

  5. Schedule the PICKUP with UPS/FedEx online. If you don’t have an account yet, please call, and we will set up your office in our system! Place the case boxes in the shipping bag and attach the shipping label to the bag.  

  6. If you have photos to send, please email them to info@dtdenture.com and reference the Doctor and Patient names so we can match them to the case.
